From September 28 to 30, Charlotte, North Carolina, hosted a vibrant ASHRM 2025 conference that exceeded expectations. Despite continuous rain, the event drew a large and highly engaged crowd. The atmosphere buzzed with energy as leaders and innovators came together, united by a shared commitment to advancing patient safety and risk management.

In September, we brought together our dedicated community of healthcare leaders for our Ideagen Healthcare Guardian User Groups for 2025. The day was built around a simple but powerful theme: to listen and improve. It was an opportunity for an open, two-way dialogue with the teams who use our system every day, including representatives from NHS trusts, ICBs, and community providers.

We had the privilege of attending the HSJ Patient Safety Congress on the 15th and 16th of September 2025 - our fourth time at this incredible event. It was an unforgettable two days filled with inspiring conversations, groundbreaking ideas, and a celebration of the remarkable achievements in patient safety across the NHS.

We want to extend a huge congratulations to Maidstone and Tunbridge Wells NHS Trust (MTW) on their fantastic win at this week's HSJ Patient Safety Awards. The trust took home the award for Safety Improvement Through Technology Award, a powerful recognition of their commitment to innovation and safer patient care.

Martha’s Rule is now officially being rolled out across all acute hospitals in England, marking a significant step forward for patient safety. This rule provides a clear way for families and carers to get an urgent second opinion if they are worried about the condition of a loved one in hospital. The move follows a successful pilot program that has already been credited with saving hundreds of lives.

This summer, Ideagen InPhase hit the road for three unforgettable Patient Safety Leadership Summits in Liverpool, London, and Birmingham, bringing together passionate healthcare leaders to explore the state of patient safety, the challenges they face, and how digital innovation, like Ideagen InPhase, can lead to meaningful improvements. Each event offered a unique, regionally rooted perspective, but the energy, collaboration, and commitment to better care was consistent throughout.

Healthcare is most effective when it’s a collaborative effort. Patients, families, and caregivers are more than just recipients of care—they’re essential partners in creating safer, higher-quality healthcare environments. This principle is at the heart of Domain 5 of the Patient Safety Structural Measure (PSSM): Patient and Family Engagement.
