We had the privilege of attending the HSJ Patient Safety Congress on the 15th and 16th of September 2025 - our fourth time at this incredible event. It was an unforgettable two days filled with inspiring conversations, groundbreaking ideas, and a celebration of the remarkable achievements in patient safety across the NHS.

Conversations That Matter

Over the course of the event, we spoke with more than 200 delegates, each passionate about improving patient safety systems. A recurring theme in our conversations was the frustration many trusts face with managing multiple different systems that don’t integrate, making triangulated reporting a logistical nightmare.

Delegates were really excited about a platform that triangulates everything from incidents and adverse events, risk, audits, policies, NICE, CAS, health and safety, safeguarding, and Calls for Concern (a key focus this year with the rollout of Martha’s Rule) into a single system with single sign-on to enable fast, data-driven improvements.

Delegates were particularly impressed by how users can organize their data by themes, such as medications, making it easier to identify trends and take action. This level of integration and customization is a game-changer for trusts looking to streamline their patient safety processes.

Celebrating Success on the Innovation Stage

A highlight of the event was our session on the innovation stage with Maidstone and Tunbridge Wells NHS Trust (MTW). Their journey with Ideagen Healthcare Guardian has been nothing short of transformative:

• Falls rate reduced from 6.59 to 3.8 per 1,000 bed days - the lowest in 5 years.
• Significant harm incidents dropped to 0.2 per 1,000 bed days.
• Cultural transformation: Reporting ease sentiment improved from 24% to 56%, with 87% agreeing incidents are now used to learn and improve.
• Efficiency: Incidents can now be reported in under 2 minutes.

MTW’s success story is a testament to the power of technology in driving meaningful change, using triangulation and dashboards to facilitate the wonderful work of MTW teams. Their efforts were rightfully recognized with the Safety Improvement Through Technology Award at the HSJ Awards for their initiative, “Safety in Real Time: Transforming Patient Safety through our Digital Quality Oversight System.”
