On 18 June, patient safety and governance leads from across the North gathered at The Deansgate Hotel in Manchester for the second Patient Safety Leaders Summit of the 2026 series. A week on from Birmingham, the energy in the room was just as strong — and the conversations were just as honest.

Let's be honest about what software can and can't do. Learning Disability Week 2026 runs 15–21 June. This year's theme – 'Do you see me?' – is a question worth sitting with if you work in residential or supported living. We want to be straightforward about something: software doesn't see people. Your staff do. The relationships, the compassion, the moment-to-moment support – that's human work, and no platform changes that.

With PSIRF now embedded in NHS practice and AI moving from conversation to practical capability, the timing felt right for the region to take stock.

On 2 June, patient safety and governance leads from across Ireland gathered at the Clayton Hotel Dublin Airport for Ideagen Healthcare Guardian's first Patient Safety Leaders Summit in Ireland. With the Patient Safety Act 2023 now in force and the 7-day notification requirement a live obligation for healthcare organisations across the country, the timing felt right. The conversation that followed confirmed it.

Chelsea and Westminster Hospital NHS Foundation Trust recently brought together over 200 nurses and midwives — from newly qualified staff through to senior leaders — for a day built entirely around professional excellence and better patient care. The agenda covered everything from high-quality care delivery and panel discussions on frontline realities, to breakout workshops on VR innovation, compassionate care under pressure, and developing quality improvement projects. We were proud to be there.

One in three NHS staff have never reported a patient safety incident (Mahajan et al., Cureus, 2025). A further 41% have missed opportunities to report.

We are delighted to announce that NHS Dorset — a collaborative group comprising University Hospitals Dorset NHS Foundation Trust, Dorset County Hospital NHS Foundation Trust, and Dorset Healthcare University NHS Foundation Trust — has selected Ideagen Healthcare Guardian as its unified patient safety and governance platform across all three organisations.

The shift finishes. Someone needs to complete the incident report before they leave. The form asks for structured information - contributing factors, harm categorization, causal detail - that requires cross-referencing records they no longer have open. They complete what they can, estimate what they can't, and move on.

A family member mentions during a visit that their relative seems cold in the lounge. The comment gets noted informally, perhaps mentioned to a carer on duty. Two weeks later, an incident report documents that the same resident has developed a chest infection. A separate environmental audit reveals heating inconsistency in that wing of the building.

A care home reviews three months of incident reports and notices several falls across different residents. Each fall was properly investigated, documented, and reported to families. But when the quality lead asks "is there a pattern here?" - finding the answer requires checking audit results, risk assessments, and recent feedback separately, across different files and formats.
