V6 Taxonomy Guide for Learn From Patient Safety Events (LFPSE)

The Learn from Patient Safety Events (LFPSE) framework is constantly evolving. Reporters are learning from reported incidents and the Taxonomy iteself is learning what the best data to collect is. This guide provides support for organisations to learn about the Taxonomy and its changes to best prepare their teams.

As of 2nd August 2023, Version 5 of the Learn from Patient Safety Events (LFPSE) Taxonomy is the current, live version that is being used. We have worked closely with NHS England on Version 6 of the Taxonomy, which has now been finalised and released.

InPhase's work to support this Taxonomy change is well underway. The earliest anticipated date for V6 going live is October 2023.

To help prepare for the upcoming changes, we have prepared this guide as a training and reference tool. This Guide helps support:

  • Training in new terminology
  • Preparation of what local questions you will want to add to or remove from the LFPSE questions in your Oversight system
  • Identification of where in the flow of questions you will want to insert the local questions to be easiest for the reporter to speed data capture and ensure staff time used is minimised.
