Case Study

SpaMedica implemented Ideagen Healthcare Guardian in 2023. After a period of use, the team recognised there was more to unlock — the initial build leaned heavily on familiar Datix-style processes rather than making full use of the platform's flexibility.

An over-reliance on manual tracking and inconsistent input methods was affecting data quality. The team found themselves spending more time on admin than on learning and quality improvement — not the goal they'd purchased the system for.

So SpaMedica set new objectives: one system across incidents, complaints, risk and medico-legal that allowed true triangulation; improved data quality; real-time oversight; and stronger learning and improvement, rather than a system simply used to log where things sat.

A project group was formed, giving the team the confidence and internal expertise to take full ownership of their patient safety data and shape the system independently.

"Engage all your stakeholders. Map who they are, get them around the table — it touches so many different parts of the organisation."
- Rebecca Aston, Patient Safety and Experience Lead, SpaMedica

The team mapped real workflows from stakeholder feedback and agreed a phased, realistically-timed rollout — module by module, with training built in at each stage, supported by expert consultancy along the way.

Ideagen Healthcare Guardian's configurability let SpaMedica shape its access model exactly to its needs — four clear roles, from input-only through to a tightly-controlled super user group, mapped directly to job role for clean onboarding and offboarding.

The platform's Question Manager gave the team the flexibility to tailor incident forms precisely to their workflow — conditional logic so questions appear only when relevant, duplicate questions consolidated, and record linking that shows how many incidents sit behind a single risk.

"I couldn't build dashboards before. I can now."
- Rebecca Aston, Patient Safety and Experience Lead, SpaMedica

SpaMedica's SWARM investigation process — previously run on a separate form — was built directly into the system using this same flexibility, so lead investigators input high-quality data once, ready for thematic analysis.

The system's triangulation, customisation and interoperability gave SpaMedica a way of working that Datix never allowed — dynamic complaint forms, automated notifications, and a live tracker replacing manual spreadsheets. Thematic dashboards revealed that attitude and behaviour was the leading concern, and that it often correlated with clinical complications, prompting further investigation.

Then AI changed what was possible. SpaMedica now records SWARM and MDT discussions in Teams, uploads the transcript, and uses AI — locked strictly to what was said in the room — to draft the investigation write-up, capturing every voice without missing detail human memory alone would.

"I've asked AI to pick out the key points of a complaint, and it's better than me. It is. It misses nothing."
- Rebecca Aston, Patient Safety and Experience Lead, SpaMedica

AI also drafts investigation reports, duty of candour letters and complaint response letters — aligned to PSIRF and SpaMedica's own values — and suggests ways to strengthen recommended actions using SMART methodology. The lead investigator remains fully responsible for every word.

The impact has been significant: incidents open beyond 30 days down from 1,153 to 109; the 20-day complaints response target hit for three consecutive months; and an 84% reduction in returning complaints.

"We've had two returning complaints across the whole organisation between January and April. We're hitting the mark first time."
- Rebecca Aston, Patient Safety and Experience Lead, SpaMedica

A small governance group reviews any proposed system changes before they go live, giving SpaMedica full ownership with a clear audit trail. Time once lost to admin is now spent on learning and quality improvement. SpaMedica is next building four new modules, including CQC and audit — extending full regulatory oversight across its 70+ hospital sites.
