Case Study
Managing governance across 35+ community services — physiotherapy, podiatry, children's services, end-of-life care and the HOPE wellbeing programme — presents a challenge that most acute trusts don't face. Staff are distributed across community clinics, patients' homes, schools and community settings rather than concentrated in one building. Incident reporting systems designed for desk-based workforces don't serve a team that spends most of its day in the field.
For Care Plus Group's five-person governance team, that challenge was compounded by the tools available. Incident reporting, audits, patient feedback and policy compliance were all managed through paper forms, spreadsheets and manual processes. Governance data arrived in batches rather than in real time. Patterns across services were difficult to identify because each service's data existed in its own silo. Time that could have been spent on quality improvement was spent chasing paper and compiling reports.
Care Plus Group selected Ideagen Healthcare Guardian as its integrated governance platform, replacing manual processes with a single digital system for incidents, audits, feedback and policies. Implementation focused on three priorities.
"We had paper-based service user feedback with a very low return rate and a very high cost. Since going digital, our return rate has at least doubled and we're getting richer data. Not only are we saving thousands on envelopes and printing, we're actually hearing more from the people we care for."
The first was mobile-first access for a distributed workforce. Staff across all 35+ services can now record incidents, complete audits and submit feedback from any device. QR codes placed in community settings provide instant access to the relevant form — removing the single biggest barrier to reporting for a team that's rarely at a desk.
The second was connected data across every governance function. Incidents, audits, patient feedback and policies all sit on one platform. When a governance lead reviews an incident, they can immediately see whether the relevant audit was compliant, whether there is related feedback from the patient or family and whether the applicable policy was followed. That connected view — data triangulation — is what transforms governance from reactive reporting to proactive improvement.
The third was real-time dashboards for a lean team. With five people supporting 850 staff, the governance team cannot spend days compiling quarterly reports. Real-time dashboards provide an always-current view of incident trends, audit compliance, feedback volumes and improvement action progress — visible to the governance team, service managers and the board without manual aggregation.
The results were immediate. Patient feedback responses increased by 75% after digitising the process and making it accessible via QR codes in community settings. Patients and families can now share their experience in the moment rather than retrospectively and the HOPE wellbeing service saw particularly strong adoption.
The shift from governance as a compliance burden to governance as a management tool became visible in day-to-day behaviour. Service managers began requesting access to their own dashboards — not because they were told to, but because the data was useful to them. The five-person governance team that previously spent significant time on manual data collection and report compilation now has that time back. The platform handles the collection and reporting. The team focuses on what the data means and what to do about it.
