The morning began on a high note, with a free breakfast that’s always a hit! Patient safety leaders from across the Midlands gathered in an intimate setting, where they could freely discuss their thoughts and experiences on the current state of patient safety. 

Reflecting on Past Experiences

Led by Rachael Penny from Learning and Solutions at Ideagen InPhase, the first workshop encouraged lively discussions about the challenges faced during significant system changes. Attendees opened up about their experiences, highlighting common issues such as communication breakdowns that led to mixed results and the apprehension surrounding new workflows. There were lots of insights as participants discussed varying levels of technical ability among staff and their concerns about integrating new systems into existing workflows.

The culture of change was a key focus

As the conversation flowed, attendees emphasized the critical role of organizational culture in implementing changes. A poor culture leads to poor results. Everyone recognized the need to involve all levels of staff in the process, fostering excitement and commitment to the changes ahead. Open feedback emerged as a vital component for customization, and attendees agreed that enthusiasm and understanding from every level of the organization is essential for successful implementation. Overcoming the fear of change and promoting the benefits of incident reporting were also key themes that energized the discussions.

Priorities for the Future

When the focus turned to the future, attendees identified crucial priorities for implementing patient safety frameworks. Timely communication, honesty, effective planning, strong leadership, and ongoing engagement were at the forefront of their discussions. This forward-thinking mindset set an inspiring tone for the rest of the event.

Learning and Sharing: Incident Reporting Workshop

Later in the day, Alistair Brown led a dynamic workshop centered on incident reporting and leveraging reports to enhance learning and improve patient safety. Attendees eagerly shared their current challenges with logging incidents, from limited access to reliable computers to cumbersome reporting forms. Participants identified key factors for effective incident investigation, emphasizing the need for accurate data, relevant documentation, and accessible analysis.

Celebrating Success: Wye Valley NHS Trust

A highlight of the event came from Natasha Owen, Associate Director of Quality Governance and Lynn Carpenter, Quality and Safety Matron, of Wye Valley NHS Trust, who passionately shared their remarkable journey in improving patient safety governance through the implementation of Ideagen InPhase. They described how the system has streamlined LFPSE and PSIRF processes, allowing them to enhance patient safety and assurance with a user-friendly and efficient platform. 

The Trust’s ability to generate customizable dashboards and reports in-house, along with their capacity to triangulate incident data with other key areas, was met with admiration. The community teams now enjoy the convenience of reporting incidents in real-time while in the field without having to wait until they return to hospital to use a ‘trusted’ computer.

Wye Valley were also able to move away from paper-based reporting in their mortality case making linking mortality cases to wider patient safety data possible and digitizing reports and drastically reduced the time required for Structured Judgement Reviews for mortalities from 12 months to just one month!

An Interactive Experience

As the event progressed, our team highlighted the power of InPhase Oversight and its associated solutions, sparking conversations around creating a positive culture around incident reporting, overcoming obstacles to change, enhancing learning around incidents and improving patient safety. The morning was filled with enthusiasm and valuable insights, creating an atmosphere that was both enjoyable and inspiring for all involved.
