The hot topic in patient safety regarding Learn from Patient Safety Events (LFPSE) taxonomy V6 and when it will be ready has raised a few questions. So, to help clarify some things, we've put together some frequently asked questions to keep you in the know.

What is LFPSE V6?

LPFSE V6 is the latest version of the LFPSE taxonomy that healthcare suppliers use to report patient safety incidents to NHS England.

Why is LFPSE V5 changing to LFPSE V6?

LFPSE is a constantly evolving framework. As the taxonomy continues to be used and the data is utilised for learning and improvement work, NHS England and healthcare providers alike may notice something that would work better or additional data they would like captured for a more in-depth view of incidents.

What is changing with LFPSE V6?

LFPSE V6 introduces several changes, including the ability to delete records without NHS England intervention, modifications to medication coding, and the inclusion of supporting questions for Patient Safety Incident Response Framework (PSIRF).

Additionally, V6 streamlines data submission to the Care Quality Commission (CQC) through replacement questions for the STEIS system. Despite these changes, reporting Risk and Outcomes events remains optional and unchanged.

Why do we not have LFPSE V6 yet?

No supplier of patient safety software can fully implement the changes to your live and in-use patient safety system without the go-ahead from NHS England. Once NHS England is ready to receive data from LFPSE V6, it will be implemented in your systems.

When will LFPSE V6 be ready for us to use?

Once NHS England gives notice that they are ready to receive the data from LFPSE V6, it will be implemented on your system prior to the deadline they provide. NHS England is communicating closely with suppliers, and InPhase currently stands to be ready to implement the change by the end of August 2024.

The guideline for the deadline is November 2024 for provider implementation and use. Specifics will be provided when they are released by NHS England.

My supplier says they won't be ready until January 2025 - what happens now?

Unfortunately, some suppliers of patient safety software have stated that they will not be ready to implement the changes until January 2025.

InPhase customers, however, are estimated to be ready for implementation of LFPSE V6 in August 2024 - plenty of time before the deadline.
